This provides an overview of the measures taken by Walsh & Associates to protect the personal health information (P.H.I.) it collects, uses and discloses. In the province of Ontario, Walsh & Associates' Personal Health Information Protection Program meets the legislated health privacy requirements of Schedule A of the Health Information Protection Act.
Collection of Personal Health Information
Walsh & Associates collects personal health information (PHI) directly from you or in rare situations, from a person acting on your behalf. The personal health information that we collect may include your name, date of birth, address, health history, record of visits to the Occupational Health Services and care that you receive during those visits. Walsh & Associates will collect only that personal health information relevant to purposes for which you have consented or if the law permits.
Limiting Use, Disclosure and Retention of Personal Health Information
Walsh & Associates will only use and disclose your personal health information in the ways for which it has your consent, except where required by law. Some limited examples would be:
Walsh & Associates will only keep your personal health information for as long as required to fulfill the purposes for which it was originally collected, or as required by law.
Consent
Walsh & Associates will only collect, use and disclose your personal health information in the ways for which you have given your consent. Where required by law, Walsh & Associates will always obtain your express consent prior to the collection, use or disclosure of your personal health information.
Individual Access
You have the right to access and correct the personal health information held by Walsh & Associates or withdraw your consent for some uses and disclosures by contacting Walsh & Associates (legal exceptions may apply).
Safeguards
Walsh & Associates will take all reasonable precautions to protect the personal health information in its possession from unauthorized viewing, destruction or alteration. To this end, security practices protect both the storage and movement of paper and electronic health records. Steps are taken to ensure that every person or agency that performs services for us protects your privacy and only uses your P.H.I. for the purposes to which you have consented.
Openness
The on-site local contact person at each location where we provide services is the Occupational Health Nurse. Should you have any questions regarding any aspect of Walsh & Associates health information protection program, you should contact your local contact person, who’s duty it is to answer those questions.
